About the role

We are seeking a Licensed Practical/Vocational Nurse (LPN/LVN) to provide skilled nursing care in patients' homes, under the supervision of a Registered Nurse. The role involves delivering and coordinating quality patient care, assessing patient conditions, documenting activities, updating care plans, and implementing appropriate nursing actions to achieve positive outcomes.

The nurse will educate patients and families, adhere to infection control standards, and ensure compliance with policies, regulations, and the Nurse Practice Act.

Responsibilities

Observe and report patient status
Document care activities
Participate in care planning
Educate patients and families
Maintain safety and confidentiality standards
Function in diverse environments, including exposure to infectious materials
Respond effectively to emergencies

Requirements

Active, unencumbered LPN/LVN license
Current CPR certification
TB test within the last 12 months
Reliable transportation
Six months of hands-on nursing experience (preferred)
Physical ability to lift and reposition patients
