About the Role
Make Every Moment Matter as a Hospice Aide (CNA/HHA)! Holisticare Hospice is seeking a Per Diem Hospice Aide to join our compassionate team, providing hands-on care that brings comfort and dignity to patients at the end of life. Working closely with our nursing team, you’ll ensure each patient’s physical, emotional, and personal needs are met with respect, empathy, and professionalism.

Responsibilities
- Follow the plan of care to ensure patient comfort and well-being.
- Attend to patient and family/caregiver requests with compassion and support.
- Assist with personal care needs, including bathing, oral hygiene, and changing bedding.
- Provide exceptional customer service to ensure patient satisfaction.
- Support families by sharing knowledge and guiding them in caring for their loved ones.
- Maintain accurate documentation in compliance with agency and regulatory standards.
Qualifications
- High School Diploma or G.E.D. required.
- Valid Certified Home Health Aide (HHA) or Certified Nursing Assistant (CNA) license required.
- Minimum of 6 months experience in hospice or skilled nursing facility preferred.
- Current driver's license, vehicle insurance, and access to dependable transportation.
Pay
$19.50 - $24.50 per hour (Average Pay Range). Final compensation will be determined based on experience, qualifications, and position responsibilities. Exact rate will be discussed at the conclusion of the interview process.
