The Hospice Certified Home Health Aide (CHHA/CNA) delivers hands‑on support to patients in their homes, ensuring dignified, meaningful care during end‑of‑life journeys.

Responsibilities

• Provide daily support with feeding, bathing, cleaning, and other activities of daily living.
• Deliver compassionate care and emotional support to patients and families.
• Travel to various patient locations to provide continuous care.
• Assist with basic medical tasks under the supervision of licensed professionals.
• Maintain accurate documentation and communication with the care team.

Qualifications

• Certified Home Health Aide (CHHA) and Certified Nursing Assistant (CNA) certifications.
• Valid driver’s license and clean driving record; compliance with state and federal driving regulations.
• Reliable transportation and willingness to travel to patient homes.
• Ability to lift up to 50 pounds.
• Pass a criminal background check, physical exam, and drug screening.

Pay

Anticipated pay range: $19 USD – $21 USD.

Schedule

PRN (Per Diem), Monday – Friday, 8:00 am – 5:00 pm.
