Weekend Case Manager (RN)

About the Role

This position involves providing patient-centered care in a home health setting, including care planning, coordination, and education of interdisciplinary teams. The nurse collaborates with physicians to develop and implement individualized care plans, utilizing clinical expertise to assess physical, psychosocial, emotional, spiritual, and bereavement needs.

Responsibilities

• Perform comprehensive assessments during Start of Care visits, evaluations, re-certifications, discharges, and supervisory visits.
• Develop and modify care plans in partnership with physicians and patients, recommending resource utilization to achieve optimal outcomes.
• Make clinical recommendations based on patient assessments and implement protocols aligned with diagnoses.
• Educate patients, families, and caregivers on disease management, community resources, and self-care techniques.
• Respond to care plan changes, determining if significant condition changes require OASIS documentation.
• Apply nursing processes effectively, demonstrating proficiency in communication, assessment, and problem-solving skills.

Requirements

• Registered Nurse (RN) license in the practicing state.
• Preferred 1-3 years of experience in home health or hospice care.
• Strong communication, assessment, and clinical decision-making abilities.
• Ability to work independently and as part of a team, maintaining confidentiality and adhering to safety standards.
