Virtual Physician - Transitions of Care (Hospitalist / Post-Acute)
About the role
For our client, we are seeking a Virtual Physician - Transitions of Care (Hospitalist / Post-Acute) to join the team of a leader in the Hospitals & Medical Centers space. This remote role will focus on delivering virtual clinical care for patients transitioning out of the hospital and into post-acute settings, with a strong emphasis on reducing readmissions and improving outcomes. You will serve as a key clinical escalation point for an interdisciplinary care team while providing oversight, coaching, and support across multiple disciplines. The position also includes driving quality improvement efforts, case review, and collaborative practice with advanced practice providers.
Location: Remote - US based candidates only, no visa sponsorship available
Responsibilities
- Deliver high-quality virtual patient care to discharged patients at high risk for readmission
- Serve as the final escalation point for clinical questions within the interdisciplinary care team
- Oversee quality and clinical outcome metrics to improve readmission rates
- Supervise and coach Nurse Practitioners through collaborative practice agreements
- Conduct weekly interdisciplinary rounds and deep-dive case reviews
- Provide clinical oversight and coaching across multiple healthcare disciplines
- Identify and drive quality improvement initiatives and strategies
Qualifications
- MD or DO degree
- Board certification in Internal or Family Medicine
- 3+ years of post-residency clinical experience
- Strong inpatient care background
- Experience managing chronic medical conditions
- Experience leading interdisciplinary teams
- Proven ability to drive change and innovation

Pay
Compensation: $280,000 – $338,000 annually
