About the Role

The APRN is a clinical role that provides exceptional medical care to patients by conducting physical exams, ordering and interpreting diagnostic tests, and developing patient treatment plans under the supervision of a physician. This role is safety-sensitive as it involves direct patient care and administering medication. The incumbent will report to the Regional Manager (or similar role) and collaborate closely with other healthcare professionals to deliver quality care, supporting team function.

Responsibilities

  • Practice medicine under the supervision of a physician using sound medical judgment and adhering to all ethical considerations; stay abreast of current technologies and industry standards.
  • Provide care for patients with acute and chronic illness in the older adult population under physician supervision.
  • Accountable for patient care and outcomes, seeking specialist consultation when needed while remaining responsible for detailed patient care.
  • Engage with hospitalists when a patient is hospitalized.
  • Meet all clinical obligations, including compliance with accrediting agencies, CLIA, drug prescription regulations, HIPAA, state and federal regulations, and safety requirements.
  • Assess, diagnose, treat, manage, educate, promote health, and provide care coordination for patients with acute and complex chronic health needs using evidence-based medicine and practice guidelines.
  • Follow all internal policies and procedures.
  • Complete operational tasks according to departmental guidelines to ensure appropriate clinical processing and documentation.
  • Maintain complete and detailed electronic records in the EMR of patients’ health care plans and prognosis, completed within 48 hours of each visit.
  • Provide patients with information to promote health, reduce risk factors, or prevent/manage disease or disability.
  • Analyze and interpret patients’ histories, symptoms, physical findings, or diagnostic information to develop appropriate diagnoses.
  • Interpret diagnostic results (e.g., blood gases, radiology, EKG) in collaboration with the supervising physician.
  • Exhibit compassionate, quality-focused, loyal, and conscientious behavior.
  • Establish and maintain effective working relationships with patients, their families, and team members.
  • Document continuing education per licensure and certification requirements.
  • Ensure patients are seen by the Physician every other visit.
  • Maintain patient confidentiality in accordance with policy and HIPAA.
  • Follow the proper chain of command for patient complaints.
  • Perform additional duties as assigned.

Requirements

  • Graduate of an accredited school of nursing.
  • Certified as a nurse practitioner with current state licensure.
  • At least 1 year of professional nursing experience, with 1-3 years of experience as a primary care nurse practitioner.
  • Prior experience working in an electronic medical records system.
  • Provide 5-star service in accordance with the Better Health Group's mission.
  • Skilled in traditional nursing disciplines of patient assessment and management.
  • ACLS/BLS certification as mandated by state.
  • Demonstrate knowledge of the rationale of appropriate patient care.
  • Ability to handle emergency situations calmly and effectively.
  • Ability to read, analyze, and interpret professional journals, technical procedures, or governmental regulations.
  • Ability to present information and respond to questions from various groups.
  • Ability to define problems, collect data, establish facts, and draw valid conclusions.
  • Excellent written and verbal communication, teaching ability, and telephone skills.
  • Working knowledge of Google Suite (Drive, Docs, Sheets, Slides) and Microsoft Office (Word, Excel, PowerPoint) for real-time collaboration.

Physical Requirements

  • Full range of body motion, including handling and transferring patients, manual and finger dexterity, and eye-hand coordination.
  • Requires standing, walking, pushing, bending, kneeling, and reaching at arm’s length and overhead in a clinic for prolonged periods.
  • Must be able to lift and move patients and medical equipment as needed.
  • Ability to lift/carry up to 15 lbs.
  • Requires corrected vision and hearing to normal range.
  • Ability to sit for extended periods.
  • Ability to operate a motor vehicle with own means of transportation.
