Responsibilities

  • Practice medicine under the supervision of a physician using sound medical judgment and adhering to all ethical considerations of the practice of medicine;
  • Keep abreast of current technologies and industry standards;
  • Provide care for patients with acute and chronic illness encountered in the older adult patient;
  • Under the supervision of a physician, accountable for patient care and outcomes and will appropriately seek consultation from specialists when needed, though will stay involved in, and be responsible for, the detailed care of the patient;
  • Engage with hospitalists whenever a patient is in the hospital;
  • Meet all clinical obligations of the medical practice: Accrediting Agency compliance, CLIA compliance, drug prescription compliance, HIPAA, State and Federal regulations, Safety Regulations, etc.
  • Assess, diagnose, treat, manage, educate, promote health, and provide care coordination and documentation for patients with acute and complex chronic health needs using evidence-based medicine and practice guidelines;
  • Follow all internal policies and procedures;
  • Accountable for the completion of all operational tasks according to departmental guidelines to ensure appropriate clinical processing/documentation;
  • Maintain complete and detailed records, electronically in the EMR, of patients’ health care plans and prognosis, completed within 48 hours of each patient’s visit;
  • Provide patients with information needed to promote health, reduce risk factors, or prevent/manage disease or disability;
  • Analyze and interpret patients’ histories, symptoms, physical findings, or diagnostic information to develop appropriate diagnosis;
  • Interpret data and diagnostic results for an appropriate course of action including but not limited to blood gases, radiology, EKG, etc. in collaboration with supervising physician;
  • Exhibit behavior that emphasizes compassion, quality, loyalty, and conscientiousness;
  • Document continuing education per licensure and certification requirements;
  • Enact proper chain of command for patient complaints;
  • Additional duties as assigned.

Requirements/Skills

  • Graduate of an accredited school of nursing;
  • Certified as a nurse practitioner with current state licensure;
  • At least 1 year of previous professional nursing experience with 1-3 years experience as a practicing as a primary care nurse practitioner;
  • Prior experience working in an electronic medical records system;
  • Provide 5 Star service in accordance with the Better Health Group's mission;
  • Skilled in traditional nursing disciplines of patient assessment and management;
  • ACLS/BLS certification required as mandated by state;
  • Must demonstrate knowledge of the rationale of appropriate patient care;
  • Demonstrate ability to handle emergency situations calmly and effectively;
  • Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations;
  • Ability to effectively present information and respond to questions from a various group of people;
  • Define work in terms of success and can be counted on to complete goals;
  • Demonstrated ability to handle data with confidentiality;

  • Diagnose and manage chronic diseases, such as Diabetes, Atherosclerotic Disease, COPD, CHF, Obesity, etc in the senior population.
