UR Utilization Review Coordinator
About the Role
The UR Coordinator facilitates care across the continuum through effective resource coordination and discharge planning, assisting patients in achieving optimal health, access to care, and appropriate utilization of resources, balanced with the patient’s resources and right to self-determination. The role requires knowledge and experience in confidentiality and compliance with HIPAA regulations.
Responsibilities
- Collaborates with physicians, therapists, the treatment team, and patients to develop effective continuing care plans.
- Makes telephone calls, sends faxes, makes copies, and coordinates discharge plans.
- Provides patients with reasonable aftercare options and discusses these individually and/or with family, as requested by the patient.
- Collects and releases information to accurately research and refer to services.
- Maintains timely and accurate records.
- Initiates contact with referral sources, community mental health providers, physicians, nursing homes, residential treatment facilities, and other placement options to facilitate continuity of care.
- Coordinates admissions to secondary hospitals, treatment centers, nursing homes, and other placements as needed.
- Schedules follow-up appointments with psychiatrists and therapists for continued outpatient treatment whenever possible.
- Actively researches and updates provider and treatment facility lists.
- Maintains an up-to-date referral, contact, and resource database.

Pay
Placement in the pay range is based on multiple factors, including but not limited to relevant years of experience and qualifications. In addition to base pay, there may be additional compensation available, such as shift differential and other special pay practices. The posted compensation is a reasonable estimate that extends from the lowest to the highest pay CMC Phoenix, in good faith, believes it might pay for this role, based on circumstances at the time of posting.
