Job Details: Schedule: Days shift, 8 hours per day, minimum of 40 hours per week. Required Qualifications: Valid Missouri State License; 1 year of Physical Therapy (PT) experience highly preferred, especially in Long Term Care (LTC) or rehabilitation settings. New graduates with Level II experience are eligible. Ability to effectively screen, evaluate, and rehabilitate patients with physical functioning disorders and wound care. Utilizes standardized assessments and evidence-based practices to support clinical interventions. Administers procedures including manual techniques, ambulation, therapeutic exercises, modalities, and wound care as part of the rehabilitation plan. Monitors patient progress, adjusts treatments accordingly, and evaluates service outcomes. Provides patient, family, caregiver, and team consultation and counseling. Maintains thorough documentation in electronic health records, including discharge summaries, treatment minutes, and clinical notes in compliance with regulations. Participates in meetings such as patient care conferences, utilization reviews, and training sessions. Supports supervision of physical therapist assistants, rehabilitation aides, and students. Adheres to the American Physical Therapy Association (APTA) and state required practice acts, conduct codes, and ethics. Able to obtain necessary durable medical equipment (DME) and supplies as required.
