Responsibilities

• Conduct utilization management activities to ensure appropriate level of care and alignment with payer requirements
• Review resident records for documentation quality, care appropriateness, and service orders; collaborate with the interdisciplinary team
• Coordinate with physicians, nursing staff, social workers, and case managers to develop and implement care plans
• Perform concurrent and retrospective chart reviews; identify barriers to discharge and recommend actions to optimize utilization
• Facilitate discharge planning and transitions of care to support efficient bed management and safe patient flow
• Document findings and recommendations clearly in the electronic medical record; maintain accuracy and compliance with organizational policies

Requirements

• Active Registered Nurse license in Pennsylvania
• 1+ year of RN clinical experience; utilization management or care coordination experience preferred
• Strong documentation and communication skills; proficient with electronic medical records
• Ability to work independently in a fast-paced long-term care environment; effective collaboration with an interdisciplinary team
• Knowledge of utilization review concepts, care coordination, and discharge planning best practices
• Availability to work 40 hours per week for the 4-week assignment
