Responsibilities
Conduct utilization management activities including chart review for medical necessity and appropriateness of services
Collaborate with an interdisciplinary team to coordinate care and ensure appropriate levels of care and timely discharge planning
Apply utilization review guidelines and payer policies to determine authorization requirements and coverage
Document decisions and clinical rationale clearly in patient records, ensuring accuracy and compliance
Provide on-call coverage as required and communicate care plans with physicians and families
Requirements
- Active Registered Nurse (RN) license in good standing
- Experience in utilization management, case management, or care coordination preferred
- Knowledge of medical necessity criteria, payer guidelines, and authorization processes
- Strong documentation and communication skills; ability to collaborate across disciplines
- Ability to work effectively in a skilled nursing setting and manage on-call duties
