- Registered Nurse – Case Management (RN Care Manager) | Travel assignment | York, Maine
- Start: 08/24/2026 | Duration: 13 weeks | 40 hours per week guaranteed
- Shifts: 8:30 a.m.–4:30 p.m. or four 10-hour shifts starting 7:30 a.m. with weekend/holiday rotation and no call
- Setting: hospital on-site

Responsibilities
- Coordinate concurrent utilization management activities across the patient’s hospital stay; serve as the primary liaison among the patient, care team, organization, and payors
- Conduct patient assessments to identify resources needed for transitions to the next setting; arrange durable medical equipment, transportation, and health insurance authorizations
- Lead interdisciplinary rounds, care conferences, and care planning; educate and collaborate with team members
- Communicate effectively in writing, verbally, and via electronic systems to ensure care coordination and documentation
- Partner with community resources to secure services and supports for discharge planning and post-acute needs
- Support length-of-stay management and apply knowledge of reimbursement methods and quality measures relevant to care management
Requirements
- Associate or Bachelor degree-prepared Registered Nurse
- Active Registered Nurse license (Maine or eligible)
- 3–5 years of clinical experience in medical/surgical nursing, home care, hospital case management, and/or utilization review
- Utilization review and discharge planning experience
- BLS required; CCM or ACM certification preferred; certification by a nationally recognized case management or utilization review organization preferred
- Ability to work in a hospital setting with flexible shifts and weekend/holiday rotation
