Location: Denver, CO 80222 | Duration: 3 months with possible extensions
About the Role
We are seeking an experienced Registered Nurse to provide skilled home health nursing services throughout the Denver Metro Area. The RN will manage patient care, perform OASIS assessments, provide wound care, oversee medication management, and educate patients and families to ensure optimal health outcomes.
Schedule
Monday-Friday, 8:00 AM - 4:30 PM with rotating on-call and weekend coverage.
Productivity Expectation: 30 Points per Week
Mileage Reimbursement: Yes (between patient visits)
Responsibilities
- Perform Start of Care (SOC) and OASIS assessments.
- Provide wound care and skilled nursing interventions.
- Manage patient caseloads and develop individualized care plans.
- Administer and monitor medication management.
- Perform catheter care and management.
- Obtain labs and monitor patient progress.
- Educate patients and caregivers on disease management and treatment plans.
- Maintain accurate and timely clinical documentation.
- Participate in rotating on-call and weekend coverage.
Requirements
- Active Registered Nurse (RN) license.
- Home Health RN experience required.
- OASIS assessment experience required.
- Strong clinical assessment and case management skills.
- Valid driver’s license and reliable transportation.

- Excellent patient education and communication skills.
- Strong organizational and time management abilities.
- Experience managing complex home health patients.
