Our client is seeking a Home Health Registered Nurse.
Schedule
- Regular hours: Monday–Friday, 8:00 AM – 4:30 PM
- On-call: Monday–Friday, 4:30 PM – 8:00 AM
- Weekend on-call: Friday 4:30 PM – Monday 8:00 AM
- Coverage area: Up to a 60-mile radius from the agency
- Mileage reimbursement: Yes, between patient visits
Responsibilities
- Perform comprehensive nursing assessments and develop individualized hospice care plans
- Provide skilled nursing care to hospice patients in home and community settings
- Monitor patient conditions, manage symptoms, and administer medications and treatments as ordered
- Educate patients, families, and caregivers on disease progression, symptom management, and end-of-life care
- Coordinate care with physicians, interdisciplinary team members, and community resources to ensure continuity of care
- Accurately document patient visits, assessments, and care plans in Homecare Homebase (HCHB)
- Respond to patient and family needs during on-call hours and provide timely clinical support
- Participate in case conferences and collaborate with the hospice team to optimize patient outcomes
Requirements
- Active Arizona RN License or Compact RN License
- Recent HCHB (Homecare Homebase) experience
- Hospice nursing experience
- Strong clinical assessment and patient care skills
