Seeking an experienced and compassionate Hospice Registered Nurse (RN) Case Manager to provide high-quality end-of-life care to patients throughout Harrison County, Mississippi.
Schedule & Work Details
- Coverage Area: Harrison County, MS
- Schedule: Monday through Friday
- Shift: 8:00 AM - 4:30 PM
- Productivity Expectation: 28 points per week
- Weekend Requirement: Participation in weekend rotation
- Mileage Reimbursement: Reimbursed from the office; mileage is automatically adjusted when starting from another location
- Dress Code: Royal Blue scrubs; nursing or healthcare-themed tops acceptable with Royal Blue scrub bottoms
Pay
$51.00 - $53.00 per hour
Responsibilities
- Manage a hospice patient caseload and serve as the primary nursing contact for assigned patients and families.
- Conduct comprehensive patient assessments to evaluate hospice eligibility and ongoing appropriateness for hospice services.
- Provide skilled nursing care and symptom management focused on comfort, dignity, and quality of life.
- Develop, implement, and update individualized hospice plans of care in collaboration with physicians and the interdisciplinary team.
- Monitor patient conditions and promptly address changes in symptoms, pain levels, and overall health status.
- Document clinical findings thoroughly and accurately to support hospice eligibility and regulatory compliance.
- Coordinate care with physicians, social workers, chaplains, hospice aides, and other healthcare professionals.
- Educate patients, caregivers, and family members regarding disease progression, symptom management, medications, and hospice services.
- Attend, prepare for, and actively participate in interdisciplinary team meetings, case conferences, and care planning discussions.
- Provide emotional support and guidance to patients and families throughout the hospice care experience.
- Ensure timely and accurate completion of all required clinical documentation.
- Participate in the agency's weekend rotation schedule as assigned.
- Maintain compliance with Medicare hospice regulations, agency policies, and professional nursing standards.
Requirements
- Active Registered Nurse (RN) license.
- Previous in-home hospice nursing experience required.
- Strong knowledge of hospice care principles, symptom management, and end-of-life care.
- Excellent clinical assessment and critical thinking skills.
- Ability to work independently and manage a field-based caseload.
- Valid driver's license, reliable transportation, and willingness to travel throughout the assigned territory.
- Strong communication, organizational, and documentation skills.
Preferred Qualifications
- Experience with HomeCare Homebase (HCHB).
- Previous hospice case management experience.
- Ability and willingness to travel longer distances between patient visits when necessary.
- Strong interdisciplinary care coordination skills.
- Experience educating patients and families on hospice philosophy and goals of care.
- Compassionate, empathetic, and patient-focused approach to care.
