Job Summary

Seeking an experienced Travel Registered Nurse (RN) to provide compassionate hospice care and case management services. This position is ideal for a nurse with strong hospice experience who thrives in a fast-paced environment and can effectively manage a growing patient census.

Key Responsibilities

• Manage and coordinate hospice patient care in home and facility settings.
• Conduct comprehensive patient assessments and develop individualized care plans.
• Monitor patient status and communicate changes to physicians and the interdisciplinary team.
• Provide symptom management, patient and family education, and end-of-life care support.
• Maintain timely and accurate clinical documentation.
• Participate in on-call rotations, including weekends and weekdays as assigned.
• Collaborate with team members to ensure high-quality, patient-centered hospice care.

Required Qualifications

• Active Registered Nurse (RN) License.
• Recent Hospice Case Manager experience.
• Strong clinical assessment and care coordination skills.
• Valid driver's license and reliable transportation.
