Position Summary

We are seeking a compassionate and experienced Registered Nurse - Home Health/Hospice to provide high-quality patient care in home health and hospice settings throughout Kanawha County. The RN will be responsible for conducting comprehensive patient assessments, completing OASIS documentation, developing care plans, and coordinating care with interdisciplinary healthcare teams to ensure optimal patient outcomes.

Key Responsibilities

  • Perform skilled nursing visits in patients' homes.
  • Complete and accurately document OASIS assessments in compliance with agency and regulatory requirements.
  • Assess patient health status and develop individualized plans of care.
  • Monitor patient progress and adjust care plans as needed.
  • Provide education and support to patients, families, and caregivers.
  • Administer medications and treatments as prescribed.
  • Coordinate care with physicians, therapists, case managers, and other healthcare professionals.
  • Ensure accurate, timely, and compliant clinical documentation.
  • Participate in on-call rotation, including every third weekend and one additional on-call day per week.
  • Maintain productivity standards of 30 points per week.
  • Follow all home health, hospice, state, and federal regulations and guidelines.

Required Qualifications

  • Current and unrestricted Registered Nurse (RN) license.
  • Current CPR certification.
  • Strong clinical assessment and patient care skills.
  • Ability to work independently and manage a daily patient schedule.
  • Excellent communication, documentation, and organizational skills.
  • Valid driver's license and reliable transportation.

Preferred Qualifications

  • Previous Home Health experience.
  • Experience with OASIS documentation and assessment processes.
  • Hospice care experience.
  • Knowledge of electronic medical record (EMR) systems.
  • Strong time-management and critical-thinking abilities.

Physical Requirements

  • Ability to travel throughout the assigned coverage area.
  • Ability to stand, walk, bend, lift, and assist patients as needed.
  • Ability to safely perform patient care activities in home environments.
