Travel Physical Therapy Assistant LTAC Job

Overview

Physical Therapy Assistant, LTAC, Lebanon, NH, starts 07/13/2026 for 13 weeks. Weekly pay range is $1,652 to $1,727 per week with guaranteed 35 hours per week. Shift is 08:00 to 16:30. This is a travel - temporary assignment in a Skilled Nursing Facility setting.

Responsibilities

• Provide physical therapy assistant services under the supervision of a licensed PT, contributing to treatment plans for geriatric patients in an LTAC setting
• Implement and document therapeutic interventions, gait training, transfers, balance activities, ROM, strength, and functional mobility programs
• Maintain accurate and timely documentation of sessions, outcomes, and any notable changes in patient status
• Monitor patient progress, adjust activities within scope of practice, and communicate changes to the supervising PT and care team
• Educate patients and families on home exercise programs, safety, fall prevention, and activity modification to support discharge planning
• Ensure infection control, safety standards, and ethical practice during all rehab activities

Requirements

• Active Physical Therapy Assistant license in New Hampshire
• Associate degree from an accredited Physical Therapist Assistant program
• CPR/BLS certification (preferred)
• Prior PT aide/assistant experience in LTAC or skilled nursing facility settings is preferred
• Ability to perform duties in a fast-paced LTAC environment and collaborate with a multidisciplinary team
