Responsibilities

• Implement and monitor rehabilitation plans for patients under supervision of a licensed PT

• Support transfers, ambulation, gait training, and use of assistive devices while ensuring patient safety

• Maintain accurate, timely documentation in patient records; adhere to infection control and safety policies

• Collaborate with an interdisciplinary team (nursing, physicians, therapy, case management) to optimize patient outcomes
