About the role
Travel Physical Therapist specializing in Skilled Nursing Facility (SNF) settings.
Responsibilities
- Conduct resident evaluations
- Develop and implement individualized treatment plans
- Document progress using electronic medical records
- Collaborate with interdisciplinary teams
- Provide resident and caregiver education
- Participate in team meetings
- Assist with program development and staff mentorship
- Deliver day-shift therapy services
- Work in long-term care/skilled nursing facilities
- Promote functional outcomes
- Ensure infection control
- Maintain licenses
- Contribute to team and facility well-being
Qualifications
- Minimum of 3 years of physical therapy experience
- Bachelor's degree (Master's or Doctorate preferred)
- Active PT license
- BLS/BCLS certification
Skills
- Proficiency in Microsoft Office
- Strong communication skills
- Ability to lift up to 50 lbs
- Physical stamina for daily activities
