Travel Therapy - Physical Therapist

About the role

Provide in-home physical therapy evaluations and patient care for individuals recovering from injuries or managing chronic conditions across multiple counties. Manage patient progress, support recovery, and ensure quality care in a home health setting.

Responsibilities

• Conduct patient evaluations
• Travel between patient visits
• Manage multiple cases
• Deliver in-home therapy services

Mileage reimbursement provided. No on-call or weekend requirements.

Requirements

• Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program
• Active Illinois PT license
• Preferred experience in home health care
• Ability to travel within service area counties and perform evaluations
