Travel Physical Therapist – Long Term Care/SNF - $2,350 per week

CoreMedical Group is seeking a travel Skilled Nursing Facility (SNF) Physical Therapist for a 13-week assignment starting September 7, 2026, in Lexington, Virginia.

Responsibilities

• Provide physical therapy services to patients in a skilled nursing facility setting.
• Develop and implement individualized treatment plans based on patient needs.
• Assess patient progress and modify treatment plans as necessary.
• Collaborate with interdisciplinary teams to ensure comprehensive patient care.
• Document patient care activities and progress in medical records.

Requirements

• Current licensure as a Physical Therapist in Virginia.
• At least two years of recent clinical experience in a SNF setting.
• Knowledge of Medicare and Medicaid regulations.
• Excellent communication and interpersonal skills.

Qualifications

• Master's degree in Physical Therapy from an accredited program.
• Passing scores on the National Physical Therapy Examination (NPTE).
• Current CPR certification.

Pay
