Responsibilities
- Manage a caseload of approximately 6 visits per day
- Conduct assessments and develop/implement plans of care
- Support geriatric patients across multiple counties
- Perform home assessments, wheelchair evaluations, and cognitive assessments
- Maintain accurate documentation
- Float within scope and provide occasional support in various counties as needed
Qualifications
- Active OT license in Georgia or a compact state
- Minimum 1 year of home health experience
- Current BLS certification
- Proficiency in Activities of Daily Living (ADLs) and adaptive equipment
- Experience with HCHB (initially) and Epic (post-10/1/2026)
