Home health travel position in the West Portland, OR area.
Responsibilities
- Provide patient assessments, therapy services, and education for diverse adult populations (neurological and orthopedic diagnoses)
- Deliver 1:1 patient care with up to 6 visits daily
- Chart via Epic EMR and Rover app
- Travel within the service area, often within a 60-mile radius
- Adhere to business casual attire and float between facilities as needed
Requirements
- 1 year of skilled home health experience
- Active Oregon COTA license
- NBCOT certification
- OASIS documentation experience
- Current BLS certification
- Reliable transportation
- Professional references
- COVID vaccination including boosters
- Minimal return-to-office (RTO) preferred; first-time travelers and local candidates welcome
Schedule
- Day shifts (8:00 AM – 4:30 PM)
- No weekend or on-call duties
