About the Role

Travels to patients’ homes within an assigned service area to conduct assessments, develop and coordinate individualized care plans, provide skilled nursing services, educate patients and caregivers, monitor progress, collaborate with physicians and interdisciplinary teams, manage documentation, and ensure continuity of care.

Requirements

• ME/compact RN license
• BLS certification
• 2 years recent home health case management experience
• Epic and Oasis experience

Qualifications

Preferred: Experience with IV therapy, tube feeds, VAC dressings, compression wraps, port access, ostomies, chest tubes, S/p catheters, and lab draws.

Schedule

• 8:00 AM – 5:00 PM
• Every 4th rotating weekend
• 3–4 days a month callback

Pay

Pay Rate: $1,800.00 – $2,500.00 per week
