About the role

Coordinates and delivers skilled nursing care in patients’ homes, assesses their medical needs, develops and updates care plans, educates patients and caregivers, monitors progress, and collaborates with physicians and other healthcare providers to ensure safe and effective treatment.

Requirements

• Active VT or compact RN license
• BLS certification
• 1+ year of recent home health and case management experience
• Oasis experience

Preferred Qualifications

• Previous travel nursing experience

Schedule

• 13-week assignment
• 36 hours per week
• 12-hour shifts, Tuesday – Saturday (day shift)

Pay

$1,800.00 – $2,700.00 per week
