RN Case Manager - Home Health

Responsibilities

• Collaborate with the patient, their family, and the physician to develop and regularly update an individualized care plan, coordinating services through ongoing interdisciplinary communication.
• Assess the patient’s clinical, functional, and emotional status, documenting progress and reporting significant changes to the care team.
• Educate patients and caregivers on self-care techniques, medications, diet, and other physician-ordered instructions, while using each interaction as an opportunity for meaningful health counseling.

Requirements

• Compact or VT & NH unencumbered RN license
• Graduate of an accredited school of nursing
• BLS certification
• 2 years of experience in Home Health settings

Pay

Pay Rate: $1,800.00 - $2,700.00
