About the role

Assesses patients, develops and coordinates individualized hospice care plans, manages symptoms and pain, provides nursing care and education, collaborates with the interdisciplinary team, and supports patients and families to ensure comfort and quality of life during end-of-life care in the home setting.

Requirements

• Active VT or compact RN license
• BLS certification
• 1+ year of home health hospice case management experience
• Oasis experience

Schedule

• 16-week assignment
• Day shifts, Monday–Friday with some weekend coverage
• 36 hours per week, 12-hour shifts

Pay

$1,800.00 – $2,500.00 per week. Pay package is based on 12-hour shifts and 36 hours per week (subject to confirmation) with tax-free stipend amount to be determined.
