About the Role

Coordinate patient care from admission through discharge, ensuring that treatments, services, and resources align with each patient’s medical needs and recovery goals. Collaborate closely with physicians, nurses, and external providers to remove barriers to care, streamline transitions, and support safe, efficient discharge planning.

Requirements

• Active TN or compact RN license.
• BLS certification.
• Minimum of 2 years case management experience in an acute hospital setting.
• Acute hospital experience within the past year with no more than a 90-day break.
• Proficiency with Midas and Meditech systems.
• Previous travel experience as an acute care case manager.

Schedule

16-week assignment • 8:00 AM – 4:30 PM, weekdays and weekends • 36 hours per week.

Pay

Pay Rate: $1,800.00 - $2,500.00 per week.
