RN Case Manager supporting colorectal surgery patients through pre-op and post-op care coordination.

Responsibilities

• Coordinate pre-op and post-op patient care
• Schedule surgeries, imaging, and follow-up appointments
• Collaborate with providers and care teams
• Document care in Epic

Requirements

• Active RN license (MN required)
• 5+ years of RN experience
• Previous RN Case Manager experience
• Epic experience required

Qualifications

• Surgical experience preferred
• Med-Surg background preferred
• Colorectal surgery experience a plus

Schedule

• Length of assignment: 12–20 weeks
• Shift: Monday–Friday, onsite
• 40 hours per week

Pay

$1,800.00 - $2,500.00 per week
