Travel Nurse - Case Management

We are seeking an RN - Case Management for a travel position in Gastonia, North Carolina. Requirements include a minimum of 2+ years of experience.

Responsibilities

• Coordinate scheduling, referrals, and follow-up care with clinical and administrative teams
• Conduct patient intake, assessments, and symptom screenings according to clinic protocols
• Communicate clinical updates to providers for timely decision-making
• Document care, patient responses, and communications accurately in medical records
• Ensure seamless patient care coordination and documentation

Schedule

• Perform 8-hour shifts, totaling 40 hours weekly
