About the role
Provide end-of-life care in patients' homes or hospice settings, focusing on compassionate, holistic, and patient-centered support.
Responsibilities
- Assess patient needs and develop individualized care plans
- Administer pain management and symptom control
- Educate families on care techniques and end-of-life processes
- Monitor patient conditions and adjust care as needed
- Coordinate with interdisciplinary teams (physicians, social workers, aides, etc.)
- Maintain accurate documentation and adhere to safety and regulatory standards
- Provide emotional support to patients and their families
Requirements
- Valid RN license
- At least one year of recent RN experience
- Minimum six months of hospice or palliative care experience
- BLS (Basic Life Support) certification
Skills
- Critical thinking and clinical judgment
- Strong interpersonal and communication skills
- Proficiency in English (written and verbal)
- Physical ability to perform patient care tasks
- Technical motor skills for medical procedures
