About the role

Provide end-of-life care in patients' homes or hospice settings, emphasizing holistic, patient-centered care aligned with hospice philosophy and standards.

Responsibilities

• Assess patient needs and develop individualized care plans
• Administer pain management and monitor patient conditions
• Educate families on care techniques and emotional support
• Collaborate with interdisciplinary teams to coordinate care
• Ensure accurate and timely documentation
• Adhere to regulatory standards and hospice protocols
• Maintain patient safety and work effectively with diverse personnel

Requirements

• Valid RN license
• At least one year of recent RN experience
• Six months of hospice or palliative care experience
• BLS certification
