Travel Home Health Physical Therapist - $2,110 per week

About the role

CoreMedical Group is seeking a travel Home Health Physical Therapist for a 13-week assignment starting on July 18, 2026, in Crestview Hills, Kentucky.

Responsibilities

• Provide exceptional patient care in a home health setting
• Work 40 hours per week, 8 hours per shift
• Assist clients in improving their physical function and mobility
• Collaborate with interdisciplinary teams to develop and implement treatment plans
• Document patient progress and communicate effectively with patients, families, and healthcare providers

Requirements

• Current licensure as a Physical Therapist in Kentucky
• At least 1 year of recent clinical experience in home health or related field
• Bachelor’s degree in Physical Therapy from an accredited program
• Passing scores on the National Physical Therapy Examination (NPTE)

Qualifications

• Strong interpersonal skills and ability to build rapport with patients and families
• Excellent communication and problem-solving abilities
• Ability to work independently and as part of a team
• Proficiency in using electronic medical records (EMR) systems

Skills

• Skilled in manual therapy techniques
• Familiarity with home health equipment and supplies
• Knowledge of Medicare and Medicaid regulations
