Assists in obtaining patient medical records, schedule patient diagnostic testing, contact patients by phone and/or written correspondence. Writes letters, mail and fax documents. Develops and maintains organized records and documentation. Essential Functions All team members are expected to be knowledgeable and compliant with Prisma Health's purpose: Inspire health. Serve with compassion. Be the difference.Requests and obtains outside medical records from various providers and/or facilitiesSchedules initial and follow up appointments. Contact patients by phone, MyChart, written correspondence regarding appointmentsFaxes or mails medical information to case managers, referring physicians, dialysis units and consulting physiciansPerforms other duties as assigned. Supervisory/Management Responsibilities This is a non-management job that will report to a supervisor, manager, director, or executive. Minimum Requirements Education - High School diploma or equivalent.
