About the role

The Transitional Support Specialist provides personal care services or accommodations to clients enrolled in the Transition to Independence Process (TIP) program. This includes planning and leading skill development groups, assisting with meal preparation, supervising the self-administration of medication, and supporting activities of daily living.

Responsibilities

  • Supervise and/or assist residents with daily living activities.
  • Supervise the self-administration of medication.
  • Affirm or prepare client resident meals.
  • Contribute to the care planning process by providing specific information and observations of residents' needs and preferences.
  • Maintain accurate, complete, and appropriate clinical records in a timely manner.
  • Perform all job responsibilities in accordance with prescribed safety and infection control procedures.
  • Organize, develop, and implement planned recreational/educational activities.
  • Complete all scheduled tasks, care of residents and the facility (including maintenance and cleaning).
  • Be available for back-up and emergency shift coverage as needed.
  • Participate in training and development programs to enhance knowledge, skills, and personal service networks.
  • Maintain courtesy and professionalism with staff and clients at all times.
  • Maintain professional boundaries in client interactions.

Requirements

  • High School Diploma or GED required.
  • Bachelor degree in human service field preferred.
  • Experience working with severely mentally disabled population or similar population preferred.
  • Experience working with severely mentally disabled young adult population or similar population preferred.
  • CPR/First Aid and NCI/CPI Certifications required within 90 days of employment.

  • Knowledge of the individualized care plan for residents and provision of support to the resident according to the care plan.

Schedule

The Transitional Support Specialist works part-time on a 2nd shift schedule, specifically Thursday 2:45-11pm, Friday 10:45-7am, and Saturday 10:45-7am. Mandatory participation in the Thursday weekly staff meeting from 3p - 4:30p and a biweekly 1-hour supervision meeting are also required.
