Transition Coordinator

Location: Gulf Park, 730 East Beach Boulevard, Long Beach, Mississippi, 39560, United States. Full-time, grant-funded position. Applicants must be authorized to work in the United States without visa sponsorship.

About the role

The Transition Coordinator coordinates day-to-day operations of transition to adulthood programs and services, including employment, volunteerism, service, independent living, leadership, advocacy, and post-secondary education. This position works collaboratively with the executive coordinator for transition and provides guidance to the transition specialist in developing, implementing, and maintaining programs, services, and activities. Responsibilities include monitoring person-centered plan development, service coordination, case management, and internships, as well as overseeing direct training and technical assistance to youth and young adults with disabilities, family members, and service providers.

Responsibilities

• Researches, reviews, and implements transition to adulthood best practices and research-based recommendations into training and technical assistance activities.
• Designs, provides oversight, and monitors quality of transition to adulthood best practice and research-based training for youth and young adults with disabilities, families, providers, and employers.
• Coordinates and provides staff support in disseminating information on best practices at state, regional, or national trainings and conferences.
• Cultivates partnerships with local, state, and national organizations, agencies, University staff, faculty, and departments to promote transition services.
• Ensures compliance with internal program policies and regulations for all transition services.
• Provides guidance to transition staff in developing schedules, program structure, and training curricula.
• Documents and tracks all transition activities, including data collection and quality monitoring, for reporting to IDS and funding agencies.
• Guides reporting and monitors implementation of transition services, including intake forms, needs assessment tools, and individual support plans.
• Monitors staff provision of advocacy support and self-sufficiency development to individuals with disabilities.
• Performs other duties as assigned.

Requirements

• Bachelor’s degree from an accredited college or university in disability studies, social work, healthcare, special education, psychology, child and family sciences, counseling, or a related field.
• Three to five years of experience or training beyond a bachelor’s degree in working with persons with disabilities, providing transition training, technical assistance, and/or direct services.

Preferred Qualifications

• Master’s degree from an accredited college or university in disability studies, social work, healthcare, special education, psychology, child and family sciences, counseling, or a related field.
• Social work licensure or case management certification.
