About the role

As a Transition Coordinator, you will play an essential role in fueling student success by providing academic, emotional, and physical services for students who require additional support to thrive in school. You will collaborate with special education teachers, social workers, and administrators to help students age 14 and up prepare for the transition from high school to successful post-secondary life. This includes working with students on job and life skills, community-based instruction, and college and career readiness. Experience with job corps programs, post-secondary academic institutions, or Serve-Safe/Pro-Start training (culinary arts) is a plus. You will also build community connections to promote school and post-secondary success for students.

You will work with a diverse group of students from varying developmental, cultural, ethnic, racial, and socioeconomic backgrounds in smaller class sizes and diverse educational settings.

Responsibilities

  • Diagnose and address learning, social, and emotional skill gaps to uncover each student’s potential.
  • Collaborate with school teams and families to deliver SESI’s evidence-based learning models and positive behavioral interventions and support (PBIS) framework.
  • Provide academic, emotional, and physical support to help students transition from high school to post-secondary success.
  • Teach job and life skills, community-based instruction, and college/career readiness.
  • Develop and maintain connections in the community to benefit students’ post-secondary success.
  • Use SESI’s instructional models, strategies, and curricula to teach knowledge and skills, nurture confidence and character, and meet the needs of all students.
  • Develop and maintain individualized, skill-appropriate lesson plans.
  • Implement behavior management and intervention strategies, including de-escalation techniques, crisis management, and physical restraint when necessary to maintain safety.
  • Maintain student documents such as individualized education plans (IEPs), behavior intervention plans (BIPs), incident reports, and high-risk evaluation forms.
  • Drive, record, and track student growth and progress toward academic, social, and emotional goals.
  • Supervise and monitor students, including transportation to and from locations within the school.

Requirements

  • Bachelor’s degree from an accredited institution of higher learning (required).
  • Valid teaching credential appropriate to the role (secondary special education preferred).
  • Commitment to learning and implementing feedback.

Pay

Competitive salary range of $55,000–$75,000 per year, based on experience, level of education, and credentials.
