TRANSITION COORDINATOR

General Function

The Transition Specialist assists consumers at risk of nursing‑facility placement—or those needing to transition out of nursing homes—by helping them obtain services and benefits that enable them to remain in, or return to, the community.

Responsibilities

• Identify services needed by consumers at risk of institutionalization to live in the community.
• Screen for Money Follows the Person eligibility for the Open Doors program.
• Work with individuals to develop an Individualized Service Plan (ISP).
• Establish bi‑weekly contact, as required by WDOM and the grantor, to ensure participants have needed services.
• Provide independent living skills training, individual advocacy, and other ongoing services as needed.
• Assist with life‑skills goals to promote independence in a community setting.
• Enter required data into the grantor’s dashboard and WDOM’s database after intake and documentation are completed.
• Visit nursing homes to conduct intake and maintain 10‑day compliance.
• Ensure deliverables are met in accordance with grantor requirements and WDOM.
• Collaborate with discharge planners, care managers, service coordinators, and other institutional and WDOM staff to secure services and support for the individual.
• Assist consumers with applications for benefit programs.
• Advocate for adequate and timely provision of services.
• Conduct outreach to community‑based organizations, service providers, and healthcare providers to promote the program and Westchester Disabled on the Move.
• Other duties as assigned.

Requirements & Qualifications

• Preferred: Bachelor’s degree in human or social services, or an associate degree with two years of relevant social‑service experience.
• Working knowledge of issues faced by people of all ages needing long‑term care services, especially nursing‑home transition, and a commitment to person‑centered planning and consumer choice (or willingness to learn).
• Basic knowledge of Medicaid eligibility and coverage.
• Excellent written and oral communication and organizational skills.
• Ability to actively listen and, when appropriate, counsel throughout the transition process.
• Strong time‑management skills to maintain outreaches, caseload, documentation, and administrative requirements.
• Ability to work independently and as part of a team.
• Computer literate with basic Microsoft Office and Internet search skills.
• Respectful of people with disabilities from diverse cultural and socio‑economic backgrounds.
• Bilingual skills preferred; fluency in a second language a plus.
• Must own transportation, have a valid driver’s license, insurance, and be able to travel throughout Westchester County.
• License/Certification: Driver’s license, working vehicle, and car insurance required.

• Ability to commute to Yonkers, NY 10701 and throughout Westchester County (required).
• Ability to relocate to Yonkers, NY 10701 before starting work (required).
• Work location: In‑person (no remote work).

Pay

• Salary: $28.00 – $30.00 per hour.
• Sign‑on bonus: $2,000 total; $1,000 after six months of satisfactory employment and $1,000 after one year of satisfactory performance.

Schedule

• Expected hours: 17.5 hours per week, Part‑time.
• Work hours: 9:00 am – 5:00 pm, Monday through Friday.
