About the Role

The STAR Kids Transition Coordinator plays a critical role in supporting individuals with disabilities as they transition from children’s programs to adulthood. This position ensures that youth aging out of STAR Kids receive comprehensive planning and support to maximize independence and qualify for appropriate adult programs, such as STAR+PLUS or other community-based services. The coordinator also works to relocate members from nursing or intermediate care facilities back to community settings of their choice.

Responsibilities

  • Assemble a transition team and develop a transition plan with activities to maximize independence and assist youth in qualifying for adult programs.
  • Develop policies, informational materials, resource guides, transition assessments, and planning tools for integration into adult services.
  • Oversee all transition activities from initial contact through final transition to STAR+PLUS or other community-based programs.
  • Be knowledgeable about children’s issues, transition activities, local community resources, Medicaid eligibility, guardianship, and programs administered by state agencies (e.g., DADS, DARS, TEA).
  • Conduct transition assessments to create continuity of care plans and checklists ensuring no breaks in service or medication availability.
  • Identify adult healthcare providers and assist in transitioning members to new providers.
  • Work with nursing facilities (NF) or intermediate care facilities (ICF) to establish permanency plans for relocating members to community settings.
  • Establish communication links, monitoring schedules, and service plans for safe relocation and post-relocation stabilization.
  • Conduct other assessments as required by the STAR Kids contract.
  • Negotiate with vendors for services/equipment and ensure appropriate referrals to agencies and assistance programs.
  • Complete documentation of members’ care plans, progress, outcomes, and case reports.
  • Produce regular status reports to track members in STAR Kids.
  • Participate in the health plan’s performance improvement activities and committees as needed.
  • Communicate with management regarding workload status and discrepancies.
  • Ensure compliance with the STAR Kids contract and all service coordination requirements.
  • Perform additional professional services as requested by leadership.

Requirements

  • Bachelor’s degree from a four-year college or university, or equivalent combination of education and experience.
  • Minimum of 3 years’ transition or community resource experience (pediatric preferred), or 1 year in managed care or case management.
  • Previous experience with the State of Texas STAR+PLUS program or similar programs for SSI and disabled children preferred.
  • Knowledge of Medicaid/Medicare criteria and guidelines, managed care contracts, and reimbursement mechanisms.
  • Proficiency in Microsoft Office.
  • Current driver’s license and insurance.

Skills

  • Strong understanding of Texas Medicaid criteria and managed care guidelines.
  • Knowledge of programs and services supporting the health and medical needs of disabled children.
  • Ability to maintain confidentiality and comply with regulatory agencies (e.g., URAC, Federal/State audits).
  • Adherence to ethical business practices and corporate compliance guidelines.
  • Experience working with individuals with complex needs and their families.
