Telephonic Nurse Practitioner (Per Diem Weekends)

About the role

We are seeking a remote National On Call Advanced Practice Clinician (APC) for our Telehealth Urgent Care program.

Responsibilities

• Provide telephonic and telehealth care, including assessments, diagnoses, and treatment plans, utilizing EMR systems.
• Manage acute, chronic, and behavioral health conditions collaboratively with physicians and specialists.
• Coordinate care transitions, review orders, and facilitate appropriate interventions.
• Deliver cost-effective, quality care while ensuring documentation compliance with CMS regulations.
• Engage in patient and family care plan discussions, demonstrating excellent customer service and communication skills.
• Participate in quality initiatives and maintain ongoing training requirements.

Requirements

• Master’s or Doctorate of Nursing Practice degree with active, unrestricted NP license and national certification (Family, Adult, Gerontology, or Acute Care).
• Prescriptive authority and ability to work autonomously if permitted in your jurisdiction.
• Proficient in electronic health records and telehealth platforms.

Qualifications

Preferred: 3+ years clinical experience, especially with seniors or in acute care settings, and knowledge of geriatrics and complex care management.

• Strong clinical judgment, patient-centered approach, and adaptability.
• Excellent communication, documentation, and care coordination abilities.
• Ability to work independently and maintain confidentiality in a remote setting.

Schedule

This role offers flexible, remote work within the U.S., with coverage expectations including holidays and a minimum commitment of 32 hours per month.

Pay

Compensation ranges from $53.00 to $79.00 per hour, plus benefits and incentives.
