Role Overview

The National On Call advanced practice clinician (APC) provides telehealth and telephonic care within a 24/7 virtual urgent care program. Responsibilities include delivering quality, cost-effective care for acute, chronic, and behavioral conditions, performing assessments, documenting in electronic medical records, and coordinating member care transitions. The role requires excellent communication skills, customer service, and the ability to work independently across various shifts, including nights, weekends, and holidays. The APC collaborates with healthcare teams, ensures compliance with regulations, and participates in quality improvement initiatives.

Qualifications & Skills

  • Active, unrestricted nursing license in the state of practice and ability to obtain in others
  • Graduate degree in nursing (MSN or DNP) and national certification (e.g., FNP, ANP, GNP)
  • DEA licensure/prescriptive authority (or ability to obtain)
  • Proficient in electronic medical records and telehealth platforms
  • Minimum 3 years of clinical experience as an APC (preferred)
  • Strong communication, customer service, and adaptability skills

Key Duties

  • Provide virtual acute, chronic, and behavioral health care
  • Perform assessments, document findings, and ensure compliance with regulations
  • Coordinate care transitions and review treatment plans for appropriateness
  • Engage in patient and family discussions about care goals and advanced care planning
  • Maintain effective communication with care teams, facilities, and members
  • Participate in quality initiatives and mandatory training
