Telemedicine Nurse Practitioner

Role Summary Provide primary care via a telehealth platform, caring for both new and returning patients for primary care establishment, acute needs, and follow-up. Collaborate with the primary care team and use centralized results to improve patient access and ensure coordinated care across the healthcare system. Key Duties Deliver primary care services through telehealth to new and established patients.Assess, diagnose, and manage acute conditions and follow-up care.Establish and maintain primary care plans in partnership with patients.Coordinate with primary care team members to support continuity of care.Review centralized results to identify opportunities to improve access and outcomes.Coordinate care across the broader system to ensure seamless patient management. Qualifications & Skills Clinical experience providing primary care.Strong patient assessment, decision-making, and follow-up management skills.Ability to collaborate effectively with interdisciplinary primary care teams.Comfort working with telehealth platforms and remote clinical workflows.Strong communication and care coordination skills.Must be located in the DC/MD/VA area.
