About the Role

The National On Call advanced practice clinician (APC) provides telehealth care, offering 24/7 virtual support to patients, caregivers, and facility staff. This remote, full-time position involves managing acute, chronic, and behavioral conditions, coordinating care transitions, and documenting in electronic medical records (EMR) while ensuring compliance with healthcare regulations.

Responsibilities

• Deliver cost-effective, quality care via telephonic and video platforms, utilizing EMR proficiently.
• Perform comprehensive assessments, document findings accurately, and manage medical and behavioral conditions in collaboration with physicians.
• Coordinate care across settings, monitor patient needs, and facilitate care plan adjustments.
• Communicate effectively with patients, families, providers, and care teams, demonstrating excellent customer service.
• Participate in quality initiatives and mandatory training, ensuring compliance with documentation and regulatory standards.

Requirements

• Graduate degree in nursing (MSN or DNP) with active, unrestricted NP license.
• Certification as Family, Adult, Gerontology, or Acute Care Nurse Practitioner.
• DEA licensure or ability to obtain; computer proficiency for documentation.

Preferred Qualifications

• 3+ years clinical experience.
• Additional state licenses.
• Familiarity with geriatrics and complex patient needs.

Schedule

Flexibility in shifts, including evenings, nights, weekends, and holidays, with remote work within the U.S.

Pay

Compensation range of $109,500–$164,000 based on experience and performance.
