Telehealth Coordinator/ Chronic Care Registered Nurse

About the role

VitalCore Health Strategies (VCHS) is an industry leader in Correctional Health Care and is seeking a Full-Time Telehealth Coordinator/ Chronic Care Registered Nurse at Howard R. Young Correctional Institution in Wilmington, DE.

Responsibilities

• Facilitate virtual medical consultations and appointments for patients requiring specialized care beyond the facility's capabilities.
• Maintain accurate, detailed reports and records, including documenting medical histories, test results, treatment plans, and communication with external healthcare providers.
• Administer medications to patients and monitor for reactions and side effects.
• Monitor, record, and report symptoms or changes in patients' condition, coordinating with external healthcare providers and ensuring necessary medical records are prepared.
• Coordinate with external healthcare providers, schedule appointments, and maintain records.
• Perform additional administrative tasks as assigned by the facility.

Requirements

• Graduate of an accredited School of Nursing.
• Active licensure as a Registered Nurse in Delaware or a multistate area.
• Possession of an active CPR certification.
• Knowledgeable about specific state laws and regulations governing nursing practice.
• Satisfactory completion of initial and annual clinical competencies.
