General Summary

The Patient Care Coordinators (PCAs) are part of a home-based care team that provides compassionate care to frail patients suffering from chronic and acute illnesses or injuries. They are responsible for answering calls, coordinating care, scheduling appointments, managing patient records, and engaging patients.

SKILLS & COMPETENCIES

  • Practices the WellBe mission: To help our patients lead healthier, meaningful lives by delivering the most Complete Care.
  • Updates and maintains all digital client records and assists in transitioning hard copy records to digital format.
  • Acts as a community resource for the patient.
  • Schedules/coordinates WellBe provider visits with patients, tests, specialist appointments, and social resources.
  • Safeguards all appropriate patient information in the EMR.
  • Provides positive customer service and treats all patients and staff with respect.
  • Prioritizes urgent patient needs, schedules Complete Care Assessments, and engages the unengaged.
  • Collaborates with the interdisciplinary team and participates in regularly scheduled team meetings.
  • Supports APC in ongoing panel management and quality of care efforts (e.g., HEDIS).
  • Completes next day visit chart prep.
  • Utilizes reporting to help providers track, meet, and exceed team goals.
  • Performs other tasks needed to accomplish team’s objectives/goals.

Job Requirements

  • Educational/Experience Requirements: High school graduate or GED.
  • Licenses, Certifications, Registrations: MA Certification - Preferred, Required.
  • Skills and Abilities: 2+ years of experience working for a healthcare practice or hospital; EMR documentation experience preferred; strong critical thinking skills; strong customer centric focus/service skills; strong computer skills, including Word, Excel, and PowerPoint; strong verbal, written, presentation, and interpersonal communication skills; supervisory responsibility: N/A.

Pay Range

$19.00/hr - $23.00/hr

Work Environment

4 days in office and 1 day Work From Home.

Travel Requirements

No travel is required.

Work Conditions

  • Ability to lift up to 20lbs.
  • Moving, lifting, or transferring of patients may involve lifting of up to 50lbs as well as assist with weights of more than 100lbs.
  • Ability to stand for extended periods.
  • Ability to drive to patient locations (ie. home, hospital, SNF, etc).
  • Fine motor skills/Visual acuity.
