Team Nurse
About the role

Reporting directly to the Practice Manager and Senior Director of Nursing, the Team Nurse RN position is responsible for performing the duties of a RN in an outpatient primary care or family medicine practice and serves as a clinical resource for the medical practice.
Responsibilities
- Collaborate with providers to identify needs for preventive care, evidence-based chronic disease management and care coordination.
- Provide general and specific healthcare education.
- Work directly with patients to identify and monitor self-management goals.
- Collaborate care with community based providers.
- Follow up on ED visits and inpatient discharges.
- Absorb and assist with exams, treatments and procedures as needed.
- Perform a direct patient care role, including Annual Wellness Visits (AWVs) and Chronic Care Management (CCM).
- Manage INR monitoring and education for patients on Coumadin.
- Administer vaccines and medications, document accurately in the Electronic Health Record.
- Schedule tests and referrals for specialty care.
- Maintain inventory and ordering management of clinical supplies and equipment.
- Ensure safe, accurate and clinically appropriate collection, storage and processing of all biologic specimens.
- Oversee unlicensed staff, provide education and training, and maintain compliance with organizational training.
- Ensure patient safety practices to prevent/eliminate risk of patient harm.
Qualifications
- Graduate from an accredited Nursing Program required.
- Bachelor's degree preferred.
- License Registered Nurse in Massachusetts required and Certificate 1 Basic Life Support required.
- 1-3 years related work experience required. New graduate RN’s considered.
- Strong interpersonal skills, strong patient communication and patient education skills.
- Advanced skills with Microsoft applications which may include Outlook, Word, Excel, PowerPoint or Access and other web-based applications.

- Customer Service: Ability to provide a high level of customer service to patients, visitors, staff and external customers in a professional, service-oriented, respectful manner using skills in active listening and problem solving. Ability to remain calm in stressful situations.
Pay
The pay range listed for this position is the base hourly wage range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law. Compensation may exceed the base hourly rate depending on shift differentials, call pay, premium pay, overtime pay, and other additional pay practices, as applicable to the position and in accordance with the law.
