Supported Housing Care Coordinator

Location: Atmore, Alabama Salary: $35,000 + Description: Supported Housing Care Coordinator Qualifications: Must have a bachelor degree, drivers license and insurable driving record. Must be organized, written and verbal skills essential, and must be able to efficiently schedule and carry out appointments. Must have computer skills and be able to effectively document all activities in an electronic record. Preferred Experience: Bachelor degree preferrably in psychology, social work, criminal justice, health care management. Experience with individuals with mental illness. Duties and Responsibilities: Provide case management services to individuals with servere mental illness. Provides needs assessment and completes case plan for consumers. Assist consumers with budgeting, housing needs, securing Medicaid/Medicare benefits, transportation, medication management, making and keeping doctor/therapist appointments. Provides services primarily in the home/community setting as well as the occasional outptatient setting when appropriate. Travel is expected around Atmore and Escambia county area as needed. Minimim productivity of 115 hours of billable services provided monthly
