Supported Housing Care Coordinator

About the Role

Supported Housing Care Coordinator providing case management services to individuals with severe mental illness in Atmore, Alabama and the surrounding Escambia County area.

Responsibilities

• Provide case management services to individuals with severe mental illness.
• Conduct needs assessments and complete case plans for consumers.
• Assist consumers with budgeting, housing needs, securing Medicaid/Medicare benefits, transportation, medication management, and making/keeping doctor/therapist appointments.
• Deliver services primarily in home/community settings, with occasional outpatient visits as appropriate.
• Travel throughout Atmore and Escambia County as needed.
• Maintain a minimum productivity of 115 billable service hours monthly.
• Document all activities in an electronic record.

Requirements

• Bachelor’s degree (preferably in psychology, social work, criminal justice, or healthcare management).
• Valid driver’s license and insurable driving record.
• Strong organizational, written, and verbal communication skills.
• Ability to efficiently schedule and carry out appointments.
• Proficiency with computers and electronic documentation.

Preferred Qualifications

• Experience working with individuals with mental illness.

Pay

$35,000 per year.
