Support Professional - Specialized Services

About the role

Deliver individualized support to individuals with intellectual disabilities or Autism Spectrum Disorder in community settings, under the guidance of the Program Coordinator.

Responsibilities

• Provide respectful, person-centered services that promote client choice and safety.
• Facilitate activities aligned with the individual’s interests and implement their treatment plan.
• Advocate for clients within the community and connect them to resources.
• Support health and wellness by monitoring well-being, administering medications, and attending medical appointments.
• Develop safety plans, support adherence to legal or court conditions, and ensure compliance with regulations.
• Maintain accurate documentation, data collection, and timely reporting.
• Collaborate with team members, share information professionally, and participate in care planning.

Requirements

• High School diploma or equivalent; experience with individuals with disabilities preferred.
• Proficiency in Microsoft Outlook.
• Valid driver’s license and reliable transportation required.
