About the role
The Department for Persons with Disabilities (DPD) at Catholic Charities, Diocese of Paterson, seeks Support Coordinators to manage support coordination services for adults with intellectual and developmental disabilities.
Responsibilities
- Manage support coordination services for each participant, including individual discovery, ISP development, plan coordination, and plan monitoring.
- Communicate effectively with supervisors, participants, and their families, other service providers, and with DDD representatives.
- Cooperate with the agency's administration and/or Division and DHS staff during inspections or investigations.
Requirements
- Bachelor’s degree or higher in any field and 1 year of experience working with adult (21 or older) individuals with developmental disabilities.
- Experience must be equivalent to a year of full-time documented experience working with adults with developmental disabilities.
- Valid NJ driver’s license.
- 40-60 WPM typing skills, office skills, intermediate to advanced computer skills, and organizational skills.
- Ability to operate office equipment.
- Travel required throughout Sussex County and beyond.
